Nestled in San Francisco's Excelsior neighborhood on a nine-acre campus, the San Francisco Campus for Jewish Living (SFCJL) is a center of excellence in providing a continuum of care. This includes the Frank Residences, offering assisted living and memory care services; the Jewish Home and Rehabilitation Center, featuring long-term skilled nursing, short-term rehabilitation, and an acute psychiatric unit specialized in mental healthcare for older adults over age 55; and the Jewish Home and Senior Living Foundation.

Grounded in the Jewish values of dignity, compassion, and community, the San Francisco Campus for Jewish Living, fondly referred to in the past as "the Jewish Home," was founded in 1871. Today, 150 years later, it continues to build upon its legacy of enriching the lives of older adults.

Position Overview:

Reporting to the Director or Health & Wellness, the Assistant Director of Health & Wellness is responsible for overseeing the delivery of care by Health & Wellness staff in both Assisted Living and Memory Care. This position ensures the effective delivery of health and psychosocial care services within the community while maintaining compliance with Department of Social Services (DSS) regulations. The Assistant Director acts as a liaison between residents, families, staff, and medical providers regarding health and wellness care coordination and delivery. This role also represents Frank Residences in professional and industry networking and outreach activities.

The Assistant Director provides daily supervision of care staff, develops and implements tracking systems to ensure quality and accountability, meets resident needs, and measure satisfaction. The Assistant Director ensures ongoing staff development and training and leads the department in the absence of the Director of Health & Wellness.

Essential Functions:

  • Manage the care team and ensure the delivery of excellent clinical care to residents. Collaborate with clinical and non-clinical staff to meet community health needs.
  • Conduct health assessments for prospective residents.
  • Ensure DSS survey readiness and ongoing compliance with DSS regulations.
  • Conduct employee performance reviews and, when necessary, disciplinary actions, in collaboration with Human Resources and in accordance with collective bargaining agreements for unionized staff.
  • Hire, train, supervise, and monitor caregivers, medication technicians, licensed nurses and other care providers. Ensure all health care operations are within DSS regulations, including incident reporting.
  • Deliver DSS-mandated in-service training programs in partnership with Administration and the HR Business Partner Manager.
  • Collaborate with the Director of Health & Wellness to audit operational systems, monitor staff compliance, ensure community safety, and drive continuous quality improvement.
  • Ensure resident assessments are up to date, compliant with DSS regulations, and reflect accurate care levels.
  • Develop and maintain care/service plans for residents, updating them based on changes in condition and regulatory requirements. Identify and anticipate changes in resident status. Conduct family care conferences and resolve resident and family concerns.
  • Oversee pharmacy services to ensure effective coordination with contracted and external providers. Monitor medication storage areas, carts, and medication technician practices for regulatory compliance.
  • Coordinate care with external healthcare professionals and organizations, including hospitals, physicians, home health agencies, hospice providers, and specialty clinics, ensuring residents receive timely and appropriate care.
  • Support the daily operations of Frank Residences by participating in the internal meetings and external community activities.
  • Coordinate with internal transportation services to ensure resident access to medical appointments.
  • Respond appropriately to emergencies following approved emergency management protocols.
  • Represent the community in ongoing outreach and networking efforts.
  • Serve as a liaison with families to address special needs, requests, or concerns.
  • Perform other duties as assigned.

Qualifications:

  • Current, active California Registered Nurse (RN) license required.
  • 3 - 5 years of clinical experience in home care management, assisted living or other post-acute care environments required.
  • Strong knowledge of State, Federal and DSS regulations.
  • Current RCFE license or willing to obtain.
  • Demonstrated leadership ability and teaching skills.
  • Ability to manage and coordinate and multi-faceted care program, including resolving on-site problems.
  • Strong decision-making and problem-solving skills with the ability to act quickly and effectively.
  • Excellent organizational, interpersonal and communication skills (both written and verbal).
  • Self-motivated, able to work independently, and capable of handling multiple tasks in a deadline-driven environment.
  • Accurate, concise, and detail oriented.
  • Proficient in using electronic health records (HER).
  • Ability to communicate professionally with colleagues, residents, family members, and all levels of staff.
  • Ability to maintain strict confidentiality.
  • Preferred: Knowledge of quality improvement methodologies, adult learning and training principles, patient safety, risk management, DSS surveys, infection control, and employee health.

Salary: $150K, DOE
